Please claim the tuition refund at school for Fall 2011 semester as follows:

1. Fill up and sign the class cancellation form which can be downloaded at http://www.slmcs.org/Register/CancellationandAmendmentForm07.doc
2. Visit Zoe at the front desk between 1:30 PM and 5:00 PM on next two Sundays (10/9/2011, and 10/16/2011).
3. Submit the cancellation form to Zoe to get the refund check (only if it matches SLMCS¡¯s database record). One refund check per family.
4. Zoe will inform the SLMCS registration personnel to update the registration record.


Note:
1. Parents: Please sign in slmcs.org to check your account balance. A negative balance indicates a refund available from SLMCS. If the amount is not what you are expecting, please see registration personnel at school before visiting Zoe.
2. The refund should be claimed as soon as possible during this semester ONLY. Due to system limitations, any request to apply the unclaimed refund to next semester will not be feasible.
